Somerset Foundation Trust
Somerset Foundation Trust work to the trianagle of care standards. This means that the client, staff and carers work together for long term success and sustainable recovery from mental health conditions.
The carer's assessment service team specialise in supporting carers of people with mental health conditions. The initial carer's assessment is followed up with further advise and support sessions, if needed. The carer's service can:
- signpost for access to benefits
- advise on taking a break or holiday (alone or with the person cared for)
- signpost to befriending services
- access advocacy
- support physical health needs
- signpost to education sessions
- signpost to peer support
- listen in times of crisis
